I have extensively used a Ron Graham Maverick to carry a North American Arms .380 under circumstances where high discretion is required.  It has worked very well.  If I were buying one today, I'd buy the slightly smaller Stingray instead.

R.J. Hedley also makes a very well thought out holster.

While I have not personally tried one yet, the DeSantis Superfly looks very interesting.

Thinking about it. If it's as cool as my Kimber then yes.

A couple of pocket holster features that I find useful:

a) Holster cut-away under the pistol grip.
Pro:  This allows for fast acquisition of the grip when the gun is in your pocket.  This an important feature for me.
Con:  It can allow the outline of your gun to be seen through your pants.  You may want to put something else in your pocket to break up the outline.  A piece of cardboard which is cut to size can work. (Tape the edges of the cardboard.)

b) Thumb-Push to quickly break the gun free from the holster.
Pro: Faster.  No fumbling to get it free.  Can use tight enough  holster to retain the gun in the cut-away holster, and still get the gun free quickly.
Con: None.

c) Holster pictures with both features: Hedley.

Those are make or break features for me.  Of the four holster pictures posted so far, Hedley would be my choice.

WS6:

A number of people prefer the belt pouch holster design for carrying a small handgun.  It's a little slower than some other holster designs in providing access to the weapon and is thought by some commentators clearly to announce "gun on board," but it does provide total protection for an expensive pistol worn outside the belt.  I can't tell which gunleather artist made the two in your photograph, but the workmanship seems to be excellent.

Ron Graham makes one of the best belt pouch rigs now offered.  He markets it as the "MUFTI" through Pale Horse Holsters. It can be seen by scrolling down at this link:

I have this holster for my Seecamp. It clips to athletic shorts that have no pockets or belt. It has been recognized as a holster only once, and that was at my favorite gun store. Otherwise it receives not even a second glance. Two thumbs up!

I should mention that Jerry Haugen of Haugen Handgun Leather also makes a superb belt pouch rig.

In fact, I have one of his belt pouches for my G-32. I don't use a belt pouch very often, but it's in my gun safe for those times when one is needed.

I agree with the Captain that only another member of the CCW community will be apt to spot a belt pouch as a gun holster. Most people who notice it will assume that it contains a PalmPilot or other such communication device.

I'd like to see this with a magnetic snap in place of the current spring snap, and a Palm or other logo stamped on it. If people saw an appropriate logo they'd jump to the wrong conclusion automatically.

I seem to recall reading a post on another forum about someone who claimed to have attached a cellular telephone lanyard to the inside of his belt pouch and let it hang out the side to help further "camouflage" the rig. That sounds like a leaf taken from the Pager Pal holster book.
